Weight Loss

I am currently looking to lose weight and finding it very difficult to do so without cutting down on my calorie intake. Essentially what you need to achieve is a calorie deficit for weight loss, that is burn more calories than you consume in the day. However due to sciatica I have been unable to work out which has caused the pounds to pile up. My desk job does not help either. For the first time in the last 11 years I am looking at meal replacement products. to achieve weight/fat loss.

Dogs

What Signals a Dog Is About to Attack?

Nearly 5 million dog bites occur in the U.S. each year, according to the American Humane Association. Dog trainer Victoria Stilwell says most dog attacks are "perfect storms of human failure that could have been prevented." So how can you tell doggy playfulness from a prelude to an attack? Ms. Stilwell, chief executive of Atlanta-based Victoria Stilwell Positively Dog Training and host of Animal Planet show "It's Me or the Dog," offers some advice on how to read dog behavior to avoid an attack on a person or other dog.

Where and When
People are usually attacked on private property by a dog they know. "It happens when the relative has brought his dog over, or it happens at a friend's house," says Ms. Stilwell. "We just let our guard down here."

Warning Signs
Obvious signs include barking, growling, if the dog's ears are flat or suddenly prick up. Some dogs salivate when they are stressed, others cower.

The tricky thing is that many behaviors that seem friendly can be misleading. "A wagging tail doesn't always mean that the dog is happy," says Ms. Stilwell. "Dogs will wag their tails when they are upset or over-aroused." Similarly, if an unfamiliar dog rolls on its back to expose its belly, it doesn't necessarily want to be petted. It might be asking you to move away.

A dog that attacks is often acting out of fear, says Ms. Stilwell. Those signs of fear may include a dog yawning, turning its head away when someone approaches, tensing up or staring.

Some dogs will slightly lift one front paw in what Ms. Stilwell calls "the anticipatory paw," because it shows that they think something negative is about to happen.

Mouth signals
Another subtle warning sign is when a dog slightly lifts its lip to show teeth. Also, flat tongues are relaxed; curved at the sides means there is tension.

Bite Response
"If a dog is on a child and you're pulling it off, you're making the wound on that child deeper," Ms. Stilwell says.

Instead, "take a coat or T-shirt and put it over the dog's head," she says. When dogs can't see, they panic and open their mouths.

Dogs Attacking Dogs
Ms. Stilwell says she always carries treats when walking her dog. "If I see a dog coming toward mine and I don't like the look of it, I'll take a bunch of treats and throw them far away," she adds. "Nine times out of 10 they go get the food."

Who Gets More Mosquito Bites

Why some people are itching and scratching while others seem unaffected? Why do mosquitoes prefer me?

I have always felt that the wretched mosquitoes have a special liking for my blood. In a given room at a time if there are 3-4 people it is me who has the maximum bites. The rash that follows too is terrible. The skin has red spots all over and I am unable to sleep due to the burning sensation from the bites.

I think I have finally got some reason as to why it has been so. My body is a little warm than others and I tend to sweat a lot.

It is peak mosquito season, and while some lucky outdoor venturers seem unperturbed by the tiny insects, others appear to be relentlessly assaulted. Scientists are trying to understand what makes certain humans more attractive to the bugs. One expert, molecular vector biologist L.J. Zwiebel, a professor of biological sciences at Vanderbilt University, weighs in.

Sweet Smell of Success

Mosquitoes find their mammalian prey through sensing the heat and carbon dioxide mammals emit. Pregnant women and larger people are sometimes the victim of multiple attacks, since they exert more energy than the average person.

Mosquitoes are also guided by their sense of smell. "Despite what my grandmother told me, I don't have sweet blood," says Dr. Zwiebel, since mosquitoes cannot distinguish blood quality. "Mosquitoes are attracted to our human odor, and that is largely a consequence of the bacteria on our skin," says Dr. Zwiebel. The "flora and fauna on our skin" also smell appetizing to mosquitoes, says Dr. Zweibel, and these can increase when we sweat or spend a lot of time outdoors.


As far as Dr. Zwiebel knows, there is no scientific basis to prove that having high cholesterol or diabetes, despite what some websites claim, will keep bugs away. Nor will taking vitamin B supplements, eating garlic or taking specific medications.



Skeeter Immunity
After 20 years in labs with the biting bugs, Dr. Zwiebel says he can stick his arm into a container of mozzies and be bit hundreds of times, yet "not have a hair out of place." Children, who have been bitten far fewer times than adults, may have stronger reactions to bites, since their immune systems haven't matured fully. But adults who are convinced they are being victimized by the bloodsuckers usually are not. "There are some people who have strong allergic responses to the bites which may make it seem like they're being bit more often, but they're probably not," Dr. Zwiebel notes.

Fighting Back

For people covered in welts and itchy wounds, Dr. Zwiebel says to stop scratching: When the mosquito bites, it leaves behind some of its saliva, a substance that contains anticoagulants and a local anesthetic, among other things. The body recognizes it as a foreign invader and sends histamines in to counterattack. Scratching just spreads the saliva and encourages the release of more antibodies, and more itching—a vicious cycle.

Those who swell up and itch a lot when bitten "should speak with their doctor about taking an over-the-counter antihistamine daily as a precaution," says Dr. Zwiebel. The bugs will still bite, but you may not notice, since the drug prevents the allergic response that turns a painless bite into an itchy nuisance.

Cleaner Is Meaner

Female mosquitoes are driven to bite you as they need a blood meal to complete their reproductive cycle and produce fertile eggs, which takes a huge boost of protein. To reduce the chances of being bitten, wash with an antibacterial soap to reduce the volume of mosquito-attracting bacteria on your skin.


Other ways to dodge bites: Avoid the outdoors at dawn and dusk, when mosquitoes are most active. Set up an outdoor fan to shoo bugs away. And wear long trousers and shirts if it's not too hot. Dr. Zwiebel also suggests spraying your clothing with repellent that contains DEET, a chemical that acts to confuse the mosquito's olfactory receptors. "It works, and until something better comes along, it's the best thing available to prevent bites," he says.

What is Panchakarma?

Ayurveda is one of the major ancient sciences of India. The very fact that it still remains a vital health-care system shows its viability and inherent strength. It has imbibed the trends of the times as it evolved through the ages. The aim of Ayurveda is two folds, to maintain and promote the health of healthy and to treat the illness of the diseased. For serving these purposes various health regimens and treatment modalities have been described. As far as the treatment methods are concerned Ayurveda has described the use of shaman (Palliative treatment) and Shodhan (Bio-Purifaction methods) methods. The latter incorporates the five modes of purification, which include Vaman(inducing emesis), Virechan(inducing purgation), Nasya(nasal insufflations and instillations), Basti(medicated enemas) and Rakta mokshana(blood letting). These five techniques of bio purification done through several procedures purify the body system by removing toxic materials from the body. These purification methods are essential components of the curative management of those diseases that are not amenable to palliative management.
 Various massage treatment methods; the use of different techniques of oil application and the use of fomentation methods are an prerequisite and adjuvant to these Panchakarma treatments. These specialized techniques have high medicinal value and are applied to treat many diseases, which are even otherwise not manageable by conventional methods of treatment.

A Short Introduction To Panchakarma

Panchakarma literally means five methods of body purification. These methods are employed in the healthy to prevent diseases and to improve the immunity as also in the sick to treat a number of diseases. These methods are also employed as a part of the Rasayana therapy (Rejuvenation) that has gained tremendous relevance over the years. There are certain preparatory methods to be done before the actual Panchakarmas are performed. They are called as Purvakarma in Ayurveda and involve internal administration of medicated oils/Ghee (Snehan) (in the form of pills), external application of oils and fomentation (Swedan) (massage).
These panchakarmas are
 1) Vaman
 2) Virechan
 3) Nasya
 4) Basti And
 5) Raktamokshan


1) Vaman: 

This can be termed as medically induced emesis (vomiting). After proper external and internal oleation of the body and fomentation specially prepared decoction are given to the patient to drink and emesis is induced. The procedure is to be done with utmost care and only under medical supervision. This procedure is indicated in the healthy in the Vasant ritu (spring season) to prevent diseases due to Kapha dosha. Vaman is highly beneficial to treat diseases of the Respiratory system like Asthma, Bronchitis, sinusitis, Allergic rhinitis and other diseases like Hyperacidity and various skin diseases like Psoriasis, Eczemas and Pemphigus.


2) Virechan

This involves purgation by Ayurvedic method. The preparatory procedures are same as Vaman. In healthy individuals the procedure is done in Sharad ritu (autumn season) to prevent the diseases arising due to Pitta dosha. Virechan is indicated in various diseases of the abdomen and a number of skin diseases.

3) Nasya 

This means instillation of medicines through the nasal route. The medicines may be in the form of powders or medicated oils/Ghee. There are various types of Nasya and they are to be performed according to the need of the patient. Nasya therapy is primarily used in treating diseases like sinusitis, rhinitis, hair fall, Epilepsy, facial paralysis and paralysis and various other psychological and neurological disorders. Nasya is also known for improving the intellect of an individual.

4) Basti

Here medicines in the form of decoction (Niruha Basti) or oils/ghee (Anuvasan) are administered through the anal route. These are called as medicated enemas. These enemas have a high medicinal value and are used primarily in treating diseases due to Vata Dosha. Basti is helpful in treating diseases like Arthritis, Backache, Sciatica, osteoporosis and diseases of bowels like Irritable bowel syndrome, ulcerative colitis and a number of other diseases. The type of Basti and the duration of treatment are to be decided according to the need of the patient. Basti has got a prime place in the Ayurvedic method of treatment and is referred as the half medicine in Ayurveda.


5) Raktamokshan

These refers to Blood letting by various methods like shira veda, Jaloka- avacharan (Leech application) etc. The process is basically meant for purification of the blood and to treat blood related disorders, skin diseases, High blood pressure and other diseases.
 The use of leech in the treatment of many skin diseases is now well known and accepted all over the world. Leech therapy is used in treating diseases like Eczema, Vitiligo, non-healing ulcers, varicose veins, pigment disorders, haematomas etc. Various scientific studies have established the efficacy of this leech therapy


Massage and Fomentation Therapies

As essential part of Panchakarma therapy and also independently various types of massage therapies and various types of fomentations have come a long way in the treatment of various diseases especially in the management of joint disorders, orthopedic, neurological, neuromuscular and psychological disorders. These procedures are used either locally to a particular part of the body or are done on the whole body. In the fomentation techniques heat is either applied in the form of vapors or dry fomentation is done with specially designed equipments. The procedures that are performed at the Special Panchakarma Center at Podar Hospital are as under: -


1) Sarvang Abhayang 

Here trained masseurs apply medicated oils all over the body in a specific manner. Swedan (Fomentation) usually follows all types of massages. This abhayang is indicated in various diseases of the muscles and the nerves and as a process of making the body physically strong and healthy.


2) Pizichil  

This a special type of Abhyanga procedure where the massage is performed with medicated oil by pouring the oils continuously on the whole body with the help of cloth pieces dipped in the oil. This method is used in the treatment of many generalized neuromuscular diseases, hemiplegia and paraplegia.

3) Sarvang Thaila Dhara

This is type of massage where the lukewarm medicated oil is poured over the body freely with the help of a bowl or specially designed pots and the masseurs keep massaging the individual. The indications remain almost the same as Pizichil, the only difference being the quantity of oil that is used. Dhara is effective in mental disturbances, hysteria, hallucinations, Rheumatism, cervical spondylosis, Arthralgias, hemiplegia and paraplegia and chronic headache.

4) Avagaha Sweda 

The patient is made to sit in a specially designed bathtub which stores decoctions of related herbs. The contents of the tub are kept hot for continuous fomentation at desired temperatures with the help of whirlpool system. The patient has to first undergo Abhayang before entering the Tub. Generalized diseases like arthritis and generalized weakness, fatigue, muscular problems, myopathies are the indications for Avgah Sweda.

5) Pinda Sweda

This is a highly specialized procedure, which is known as Navarakizhi in Kerala. In this process swedan is done with the help of specially made bundled warm medicinal puddings made up of shali type of rice (Navara). Before the swedan medicated oil is applied to the whole body. At the end a bath with warm water or medicated water is given. Pinda sweda is highly beneficial in restoring the strength of the lost muscles due to diseases like lower motor neuron disease, paralysis, GB syndrome, muscular dystrophy and a number of other neuro-muscular diseases.

 6) Patra Pottali sweda 

In this procedure the whole body or a selected region is made to sweat by application of heat by heated bundle of specific fresh leaves of a few herbs like castor plant, Adhatoda plant, Arka patra warmed with medicated oil accompanied by massage. Similarly when the powders of herbs like Ashwagandha, Bala moola, Shatavari etc. are used to make pottali, it is termed as Churna Pottali Sweda(Navarki)
  

 7) Nadi sweda

This type of fomentation is usually done for localized conditions like Osteoarthritis, sprain, or to relieve some localized pain as in Cervical spondylitis. Here fomentation is done with the vapors generated out of medicated decoction that is then applied to the affected area with the help of a rubber tube. Proper oleation of the affected part is done prior to Nadi Sweda.
  
 8) Sarvanga Vashpa sweda(Peti sweda) 

After proper application of oil and a good whole body massage of any of the above types, the individual is asked to enter in a specially devised chamber (Peti). In case of Peti sweda fomentation is done with the help of vapours generated out of decoctions. These procedures are applied for the generalized fomentation of the body.

  

 9) Kuti sweda 

Kuti Sweda is a dry type of Fomentation where steam is not at all applied. Dry heat is applied in this type. Here a specific temperature is attained electrically with the help of high voltage heaters in a chamber and the patient is made to sit in the chamber till adequate perspiration is achieved.
  

 10) Valuka sweda/Pottali sweda 

In this method heat is applied to the affected part by with the help of a pudding filled with heated sand or medicine. Valuka sweda is used to relieve localized pain especially in cases of rheumatoid arthritis. 
 
 11) Shiro dhara


In this procedure medicated oils or decoction are poured slowly from a distance over the forehead with specially designed Shiro-Dhara Yantra. The oil is kept in a special vessel called shirodhara vessel (Shiro dhara patra). The procedure is highly useful in several psychosomatic disorders, in mental disturbances, hysteria, hallucinations and chronic headache. Shiro dhara is also effective in the management of Epilepsy, to improve concentration, in stress-induced conditions and to improve the intellect. This is also highly useful for mental retardation and in case of poor performers at school.
  

 12) Shiro Basti 


This procedure is widely used these days for the purpose of de-addiction of alcohol as also to treat various psychological disorders. The indications also include Vata disorders, facial paralysis, Insomnias, Cataract, Chronic headaches and Spondylitis. . Highly indicative in trigiminal neuralgia, hemicrania, optic atrophy, otalgia, deafness and all disorders affecting cranial nerves. 
 Here bearably hot oil is retained on the clean shaved head up to around one inch above the hair root. A leather belt tied around the head and is sealed from within with black gram paste to retain oil. This is normally carried out for around 30 minutes. This is followed by gentle massage over back, shoulder, neck, head and mandible. Shiro Basti is normally given for 7days and can be repeated if required.

  

13)  Udvartan


Udvartan means massage with the help of powders. External application of medicinal fine powders are accompanied by massage, on the whole body. Various medicinal powders are used according to the body constitution of the individual. The procedure is generally carried out for reducing the excess fat and for improving the complexion of the skin.
 Along with the above-mentioned procedures there are other specialized methods of treatment meant for specific organs. Netra tarpan, Kati Basti, Hridaya Basti are the procedures where medicated oils are kept on the respective organs by constructing a special platform around the surface location of that organ. They are applied to offer strength to the organs and to treat related disorders.

Sciatica

Sciatica is a general term for pain originating from the sciatic nerve. Sciatica is a symptom of a disorder that causes mild to sharp and sometimes excruciating pain. Patients have described sciatic pain as mildly disturbing to burning, aching, deep, and similar to a sudden bolt of lightning. Sciatic nerve pain travels from the buttock, down the back of the thigh, and into the leg. Leg pain is the classic hallmark of sciatica. Low back pain may accompany sciatica, and some patients experience sciatic pain extending into the foot. Another characteristic of sciatica is it usually affects either the lower left or right side of the body.

Sciatic pain can make life miserable. Walking, standing, bending over, driving a car, working at a computer, catching up on household chores, sneezing or coughing, and many other activities of daily living can cause sudden and intense pain. Patients who suffer sciatica, especially of a more acute nature, find the symptoms disrupt many aspects of their life.

One reason the sciatic nerve causes so much pain is because it is the longest nerve in the body! The nerve starts at the back of the pelvis and runs downward through the hip area and buttocks into each leg. Near the knee, the sciatic nerve divides into two nerves—the tibial and peroneal nerves. The tibial nerve runs behind the knee and the peroneal nerve runs along the side of the calf and ankle. Through the tibial and peroneal nerves, the sciatic nerve innervates ('stimulates') the action of many muscles in the lower legs and enables feeling in the thighs, legs, and feet.

Besides pain, other symptoms may accompany sciatica. These symptoms include sensations such as tingling, pins and needles, burning, numbness or muscle weakness. Such symptoms may be felt in the buttock, thigh, behind the knee, calf, ankle, and sometimes the foot.

CAUSES OF SCIATICA

It's important to understand which conditions cause sciatica because sciatica is not a condition, it's a medical term used to describe symptoms caused by other low back conditions.

Several low back conditions can cause sciatica, including:

Bulging disc or herniated disc: This is the most common cause of sciatica. When a disc bulges, the gel-like center (nucleus pulposus) pushes against the outer wall of the disc (annulus fibrosus). A herniated disc means that the nucleus pulposus breaks through the annulus fibrosus. With both bulging and herniated discs, the disc material can press on nerve roots, causing sciatica. The consequences of a herniated disc are worse than those of a bulging disc. The disc material that leaks out contains an acidic, chemical irritant (hyaluronic acid) that causes nerve inflammation.
Degenerative disc disease (DDD): Sciatica is a common result of the aging process that affects discs called degenerative disc disease. DDD is a change in the disc shape and function, and it can result in a bulging disc or a herniated disc and pain.
Piriformis syndrome: The piriformis muscle, located in the lower part of the spine, connects to the thighbone, and helps you rotate your hip. The sciatic nerve runs under the piriformis muscle, so muscle spasms can compress the sciatic nerve. It may be difficult to diagnose and treat as it is not easily identified by x-ray or MRI.
Pregnancy: The extra weight and pressure on your spine caused by pregnancy can cause compression of the sciatic nerve. The symptoms will usually go away after childbirth.
Spinal stenosis: Your nerves travel through passageways in your spine, and when this space narrows (stenosis), it can put pressure on the nerves in your low back.
Spinal tumors and spinal infections: Although very rare, tumors and infections can compress the sciatic nerve.
Spondylolisthesis: If a vertebra slips forward over the vertebra below it, it's called spondylolisthesis. This slip can pinch the nerve root and cause sciatica.
Trauma: You can develop sciatica because of direct nerve compression from an outside force. For example, you could be injured in a car accident, resulting in a pinched nerve in your low back.

DIAGNOSIS

Sciatica is diagnosed by physical examination, neurological testing and patient history. Generally if a patient reports the typical radiating pain in one leg as well as one or more neurological indications of nerve root tension or neurological deficit, sciatica can be diagnosed.

The most applied diagnostic test is the straight leg rising test, which is considered positive if pain in the sciatic nerve is reproduced with between 30 and 70 degrees passive flexion of the straight leg.

Diagnosis of sciatica through imaging can be achieved either with computerised tomography (CT scan) or with magnetic resonance imaging (MRI).  MR neurography may help diagnose piriformis syndrome, another cause of sciatica that does not involve disc herniation.

AYURVEDA MANAGEMENT

Ayurveda is specialized in treating Sciatica with highest treatment success rate. It offers excellent Panchakarma therapies along with internal medicines for treating Sciatica.

Ayurveda described sciatica as Gridhrasi (Gridhra means Eagle), as the gait of the affected person resembles that of an eagle’s walk and the inflamed nerve appears like eagle’s beak.

Ayurveda categorized Sciatica as one of diseases caused by vitiation Vata (one of the principle dosha in the body, responsible for the movement and functionality of the body). Sometimes even kapha (dosha responsible for lubrication and bodily fluids) vitiation along with Vata (vata kaphaj) also causes sciatica.

Treatment in Ayurveda is aimed at restoring the equilibrium through correction of the underlying functional in-equilibrium. Ayurvedic treatments for Sciatica concentrate on bringing back the aggravated vata or vata kapha to the state of equilibrium and thereby to the state of health.

Treatment comprises of three approaches, Elimination (Sodhanam) of the accumulated toxic products of digestion, metabolism and the disease process, Pacification (Samanam) and correction of the entities responsible for altered functioning and Rejuvenation (Rasayanam) of the bodily tissue to regain and maintain natural strength and vitality.

The strength of Ayurveda in the area of spine and joint treatments is globally appreciated. Since it addresses the root cause of the issue the results are fantastic.

Therapies like Dhanyamla dhara, Abyanga swedam, Pathrapotali swedam (Elakizhi), Sarvangadhara (Pizhichil), Kadeevasthy, Navarakizhi, Vasti (the most important procedure in Ayurveda for curing Sciatica permanently) etc., are done as per the necessity and condition. These therapies are directed towards relieving the inflammatory changes and underlying causes of Sciatica, releasing the spasms and nerve compressions in the affected area, strengthening and nourishing entire spine & supporting tissues. Usually the treatment period is 4 – 5 weeks according to the severity of the disease.

Along with these therapies, Ayurveda has also described Sira vedana (Venesection) and Agni karma (Heat therapy) for treating Sciatica.

SCIATICA, Gridhrasi in Ayurveda, is a Vata roga, characterized by Sharp Shocking pain radiating from the Hip unto Thigh, Calf or the Toes in one or both the Legs. Numbness, Weakness, Tingling sensation & Discomfort along the path of the Sciatic Nerve are the results of Compression, Irritation & Inflammation of the Sciatic Nerve. The most common causes are Lumbar herniated disc, Lumbar Spinal stenosis, Degenerative disc disease, Aging, Osteoporosis, Spondilitis, Pelvic Infections, Tumors, Diabetic neuropathies etc.
In the later & complicated stages of Sciatica, the gait of the affected person resembles that of an Eagle's walk & the inflamed nerve appears like Eagle's beak, hence the term Gridhrasi.
Majority of these patients give a history of lifting heavy weights on their head or back, adopting improper spinal postures while sitting or bending forward, riding two wheelers on bumpy roads, female patients standing for a longer time in kitchens - these all contribute to the unbearable & excruciating pain called Sciatica.
Ayurveda believes that Sciatica/Gridhrasi is caused by the aggravation of vayu. Sometimes the vitiation of kapha along with vata brings on the attack. At times constipation precipitates or aggravates an attack of sciatica.
Ayurveda prescribes the perfect prescription for Gridhrasi. It includes:
Dietary Regimen: Pulses, beans, and fried foods should be prohibited. Curds and other sour substances should be avoided. Saffron, in small quantities, should be mixed with milk and given to the patient to drink.
Life style Regimen: Gentle exercise of the leg is advised. Exposure to damp and cold should be avoided. Swimming in warm water is a good exercise for legs. The patient should use a hard bed and wear a lumbo-sacral belt. Yogic exercises like Halasana, Shalabhasana, and Bhujangasana may be undertaken after pain has subsided to prevent recurrence of sciatica.
